Evaluate Your Service Quality

They will help evaluate the quality of services your loved one receives and provide feedback and follow-up to help your loved one achieve their goals.
Develop Your Quality of Life Plan

A Case Manager will guide GoodLife’s person-centered planning process, called the Quality of Life Plan.
Help Build Your Individual Education Plan

Participate in the Individual Education Plan for those still in school and collaborate with school representatives on behalf of your loved one.
How to Get Started with Case Management
1. Contact your local Community Developmental Disability Organization (CDDO) to review the list of Targeted Case Management agencies, including GoodLife, who are accepting new referrals.
2. Choose a Targeted Case Management agency by informing the CDDO of your selection and completing and signing a Choice Form.
3. If you choose GoodLife, a CDDO representative will contact us, inform us of your choice, and provide us the with required documentation.
4. The GoodLife Targeted Case Manager assigned to your county will then contact you to begin services.
