Targeted Case Management

What is Targeted Case Management?

Targeted Case Managers (TCMs) make sure that those we serve have support and assistance in accessing the services they need the most.

Why Do I Need a Case Manager?

Case managers provide leadership, advocacy, and expertise to ensure the individuals they serve connect with and benefit from the services and supports they need.

Your Case Manager is your guide to help you navigate the complex system of Disability Services.

2. Choose a Targeted Case Management agency by informing the CDDO of your selection and completing and signing a Choice Form.
